Telehouse has 2 independent external power supplies, plus its own generator. It's highly unlikely they would all fail at once. I don't believe the exact cause of the incident was ever made public, but apparently there was a switch which changed over to a new power supply when the current supply went broke. This switch acted as a SinglePointOfFailure. I tell the story to illustrate how hard it can be to truly eliminate these things. (Of course, the Telehouse guys were aware of the potential problem and tested the change-over switch carefully. According to rumor, an engineer was engaged in testing the switch at the very time the incident happened...) ''Is this a good example of a SinglePointOfFailure? It was due to the switch which selected the power supply failing. There was only one switch. So I think the switch failure is a good example.
